If he's using family shared accounts, then it's actually possible to keep him banned if the server has something to look the ID. (yes, steam has api for getting the original SteamID from a family share ID)

A mix of the second and third system sounds like a good option considering there have been people in the past who will get up like 3 strikes and leave the server for the day and come back the next day to minge again.
